Instrumentation
The size and composition of a marching band can vary greatly. Some bands have fewer than twenty members, and some have over 500. American marching bands vary considerably in their instrumentation. Some bands omit some or all woodwinds, but it is not uncommon to see piccolos, flutes, soprano clarinets, alto saxophones, and tenor saxophones. E♭ clarinets, alto clarinets, bass clarinets, and baritone saxophones are less common, but can be found in some bands. Bassoons and oboes are very seldom found on a field due to the risk of incidental damage, the impracticality of marching with an exposed double reed, and high sensitivity to weather.The brass section usually includes trumpets or cornets, French horns, alto horns, or mellophones, tenor trombones, baritone horns or euphoniums, and tubas or sousaphones. E♭soprano cornets are sometimes used to supplement or replace the high woodwinds. Some especially large bands use flugelhorns and bass trombones. Specially designed versions of the lower brass have been created for use while marching. These are typically wrapped in such a way that allow the bell to face toward the audience at all times. Bands may also modify their instrumentation to remove slide trombones completely and replace them with another instrument, such as a valved trombone or marching baritone horn.
Marching percussion (often referred to as the drumline, battery, or back battery) typically includes snare drums, tenor drums, bass drums, and cymbals and are responsible for keeping tempo for the band. All of these instruments have been adapted for mobile, outdoor use. Marching versions of the glockenspiel (bells), xylophone, and marimba are also rarely used by some ensembles. Historically, the percussion section also employed mounted timpani that featured manual controls.
For bands that include a front ensemble (also known as the pit or auxiliary percussion), stationary instrumentation may include orchestral percussion such as timpani, tambourines, maracas, cowbells, congas, wood blocks, marimbas, xylophones, bongos, vibraphones, timbales, claves, guiros, and chimes or tubular bells, concert bass drums, and gongs, as well as a multitude of auxiliary percussion equipment. Drum sets, purpose-built drum racks, and other mounted instruments are also placed here. Until the advent of the pit in the early 1980s, many of these instruments were actually carried on the field by marching percussionists by hand or on mounting brackets. Some bands also include electronic instruments such as synthesizers, electric guitars, and bass guitar, along with the requisite amplification. If double-reed or string instruments are used, they are usually placed here, but even this usage is very rare due to their relative fragility. Unusual percussive instruments are sometimes used, including brake drums, empty propane tanks, trashcans, railroad ties, stomping rigs, and other interesting sounds.
A rare inclusion in a marching band that is becoming more popular is the use of Electrophones in a marching band. The most common electric instrument seen is a bass guitar, but some schools also use keyboards and lead guitar. To make the electric instruments usable, external power in the stadium is normally used, but some groups may use a car-battery mechanism that requires a car battery and a converter to give the instruments and amplifiers remote power. Most bands will have the guitars in the pit with keyboard and auxiliary percussion, but some guitarists do march in the field show. This is made possible by the use of wireless transmission systems that allow the guitarists to hook a wireless transmitter to their instrument, while a receiver is hooked to the amp. This allows free motion of the guitarists to a fixed range of distance between the transmitter and receiver. This implementation of equipment can also be used in parades. Some band scores have parts for bass guitar, but in the event that a score does not, the bass guitarist will normally play tuba/sousaphone, bassoon, baritone-bass clef, euphonium-bass clef, or trombone music. It is very rare for a score to have a part for electric guitar, but the guitarist may use oboe or flute music in this situation.
Auxiliary
Many bands have auxiliaries that add a visual component to the performance. For ceremonial bands, this could be a traditional color guard or honor guard. For drum & bugle corps and corps-style field bands, this could include Dance lines, majorettes, Auxiliary units may be collectively referred to as color guard or visual ensemble.Auxiliaries may perform as independent groups. In the early 1970s, color guards began to hold their own competitions in the winter (after the American football season, and before the beginning of the summer drum and bugle corps season). These became known as winter guard. There are also numerous dance competitions in the off-season.
The color guard of a marching band or drum and bugle corps may contain sabers, mock rifles, and tall flags. In modern bands, other props are often used: flags of all sizes, horizontal banners, vertical banners, streamers, pom-poms, even tires, balls, and hula hoops or custom built props. The color guard may also employ stage dressing such as backdrops, portable flats, or other structures. These can be used simply as static scenery or moved to emphasize block drill, and are often used to create a "backstage" area to store equipment and hide personnel.
While military color guards were typically male, band color guards tend to be primarily female, though it is becoming more common for men to join as well. A few independent units are all-male. Guard members nearly always wear a special uniform or costume that is distinctive from that of the band, not necessarily matching in design or color. The men's and women's guard uniforms are usually designed in one of two ways: nearly identically, but with gender-specific parts (i.e. skirts) adapted for the use of the opposite sex; or complimentarily, with the two uniforms designed similarly but with variations in color or form. The color guard uniform, especially in a high school marching band, need not be in school colors; in fact, they rarely are. These uniforms are designed to represent a certain aspect of the halftime show, characterize the guard members through costumization, or tell some sort of story, and can thus be in any design or color (a surprisingly common complaint among the high school audience is that guard uniforms and equipment "aren't school colors").

The size of the band may not only determine how many drum majors there are, but how many section instructors are needed as well. Section instructors function like the music director, but are mainly responsible for teaching members of a given section. Because they are commonly previous members of the section they teach, they're able to provide better instruction to combine the needs of the show with the characteristics of the given instrument.
As bands require leadership from within as well as from without, section leaders will usually be selected from among the members of each instrumental section (a "section" comprises all the band members who play the same musical instrument). The section leader is always an experienced band member, and is usually selected by the band director (rather than elected or self-appointed) for his or her leadership skills and experience. The section leader is responsible for the minute-to-minute instruction of his or her section members, and reports to the drum majors and the band director.
The director provides general guidance, selects the repertoire, interprets commentary and evaluations from judges, and auditions and/or recruits prospective members. What content is not provided by the director may be contracted from arrangers (who compose original works or adapt existing works) and copyists (who reproduce the parts of the score), choreographers, and drill designers (primarily for field bands). With the assistance of section instructors, the director also teaches performance technique—musical, martial, and visual—and assesses the pool of talent, choosing leaders and soloists as needed. The director also selects venues for public performance and oversees the staff that help provide funding and equipment. Many opportunities for member improvement are present: the director may organize clinics with various professionals, send representatives to specialty schools or camps, and/or plan trips abroad for education or exhibition.
Large bands also require a number of support staff who can move equipment, repair instruments and uniforms, create and manipulate props used in performances, and provide food, water and medical assistance. Additional staff may be utilized when the band hosts functions such as competitions and reviews. In high school bands, these activities are usually performed by volunteers, typically parents of band members or the band members of the lower grades. These people are often referred to as "runners" or "boosters". Significant support staff for college bands and independent corps are typically paid by the university or the corps organization, respectively.

Corps Style bands
Corps Style bands directly reflect the trends seen in modern drum and bugle corps of Drum Corps International (DCI). Unlike the military band style, drum corps style step sizes are constantly changing to accommodate the differing forms the band is creating on the field. Forms may be linear, curvilinear, or scatter. Music selections can include anything from symphonic music, film scores, to world music, jazz, rock, or pop music.Unlike a Military band's usage of baton twirlers, Corps Style bands may include the usage of a "color guard" which spin flags, rifles, sabres, and may also incorporate dance into their routines. Other visual elements unique to the corps style is the usage of props, backdrops, and even costuming - with the purpose of adding more theatrical elements to the show.
Other elements unique to the Corps Style bands are the incorporation of the "front ensemble". This includes the usage of keyboard percussion such as: xylophone, marimba, and bells; and/or any other color percussion instruments such as: timpani, cymbals, conga, tambourine, triangle, drum set, etc. The front ensemble may also use sound amplification or electronic instruments such as synthesizers.
The "Corps Style" is constantly evolving, with contributions coming from college bands, high school bands, and drum and bugle corps throughout the United States. Typically, corps style bands are gearing their performances for marching competitions and marching festivals. Competitions featuring the corps style on the national level include: Drum Corps International; Winter Guard International; and Bands Of America.
Show bands
Show Band, more commonly known as Traditional Style Band, refers primarily to marching bands geared towards crowd entertainment that perform on fields (for example, a football field). Typically, they perform a routine before the game, another at halftime, and sometimes after the game as well. Competitive show bands perform only one show that is continually refined throughout a season, while bands that focus on entertainment rather than competition usually perform a unique show for each game. These shows normally consists of three to five musical pieces accompanied by formations rooted in origin from Patterns in Motion, a book penned by one-time Michigan State University Spartan Marching Band assistant band director William C. "Bill" Moffit, who would later become bandmaster of Purdue University All-American Marching Band and University of Houston Spirit of Houston. Depending on the band, though the show could be practiced and completed before the football season at band camp but mostly this is only done by competition show bands.There are several varieties of Traditional Style Bands, the most recognizable would be the type depicted in the film Drumline. This style of band is based upon an amalgamation of traditional styles from bands in the Big Ten and African American traditions, where the music selections are largely based on rhythm & blues and contemporary popular music. Band members are also known to include a smaller dance routine while standing in their forms instead of marking time. Many of these types of show bands may have both a twirler line and/or a dancer line.
Other Show Bands are semi-military and semi-corps style, such as some of the Big Ten bands. These bands perform a show that is designed to entertain the audience, but feature more traditional symphonic styles of music rather than contemporary music (marches, film scores, jazz, or older pop music).
Most show bands of either type include the traditional military band instrumentation of woodwinds, brass, and battery percussion. Some also include the front ensemble keyboard percussion, and may also incorporate the use of a color guard for flag and rifle routines.
Carnival bands
Carnival bands are a UK variant of show bands. Carnival bands typically march in time to the music, and may also participate in parades and competitions.. They contain brass and percussion, but may or may not use woodwinds.[5] In The United States, specifically Southern California, carnival bands (known as Parade bands in the states) perform regularly in parades and parade competitionsScramble bands
Scramble bands (also referred to as 'Scatter' bands) are a variation on show bands. Military bands and field music

Active duty military marching bands often perform in parades with other military units and march in the same manner as other military personnel. Due to a lack of appreciation, competition venues, and military personnel, almost all military marching bands have disappeared from schools in the United States; notable exceptions the Fightin' Texas Aggie Band from Texas A&M University, the Highty-Tighties of the Virginia Tech Corps of Cadets, and the Cadets of Norwich University Military College of Vermont, the oldest collegiate band in the United States and the nation's first private military academy. There is also a pocket of about 80 high school military-style marching bands in Eastern Texas which enjoy a strong cult following.
Unlike in the United States, military style marching bands have a strong presence in Latin American countries, especially those that have strong military traditions, most importantly of Prussian, French, Spanish and Portuguese origins. Such bands (military bands and Corps of Drums) are found in Chile, Argentina, Bolivia, Brazil, Uruguay, Colombia and Peru. But in Ecuador and Venezuela, Corps of Drums are the main military style band for schools. Military style Corps of Drums are also seen in schools, colleges and universities in Mexico and in Mexican schools in the US merely using only the snare drum and the bugle (in some schools the instrumentation can be larger). These bands also are present in Guatemala, Belize, Nicaragua, El Salvador and Panama.
The United Kingdom keeps the military style tradition with many civil and youth bands in all of the UK keeping the military band traditions of the country, either as marching wind bands, Corps of Drums, bugle bands, pipe bands, and in Northern Ireland, fife and drum bands. Examples would be the Royal British Legion bands, and the bands of the various UK youth uniformed organizations. Styled on the UK military tradition, Fiji's Military Forces also have a marching band.[4] Military style marching bands are also commonplace in Germany, Austria, the Russian Federation, Ukraine, Belarus, Italy, Spain, Portugal and France. German and French military style band types can even extend to Corps of Drums and fanfare bands staffed full-time by civilian musicians, and even to the Bagad bands in Brittany and outside France made up of bagpipers. Spanish band traditions include military style brass bands, marching bands (using brass, woodwinds and percussion) and bugle bands and the Asturias region's unique pipe bands.

TINJAUAN TEORI
A. Definisi
Herpes zoster adalah radang kulit akut yang bersifat khas seperti gerombolan vesikel unilateral, sesuai dengan dermatomnya (persyarafannya).
Herpes zoster adalah sutau infeksi yang dialami oleh seseorang yang tidak mempunyai kekebalan terhadap varicella (misalnya seseorang yang sebelumnya tidak terinfeksi oleh varicella dalam bentuk cacar air).
Herpes
zoster adalah penyakit yang disebabkan oleh reaktivasi virus Varicella –
Zoster yang sifatnya localized, dengan ciri khas berupa nyeri
radikuler, unilateral, dan gerombolan vesikel yang tersebar sesuai
dermatom yang diinervasi satu ganglion saraf sensoris.
Herpes simpleks adalah infeksi akut yg disebabkan oleh virus herpes simpleks (virus herpes hominis) tipe I atau tipe II yang ditandai oleh adanya vesikel yang berkelompok diatas kulit yang sembab dan eritematosa pada daerah dekat mukokutan, sedangkan infeksi dapat berlangsung baik primer maupun rekurens
Penyakt
infeksiosa dan kontagiosa yang disebabkan oleh virus herpes simplek
tipe 1 dan 2 dengan kecenderungan menyerang kulit-mukosa (orofasial ,
genital), terdapat kemungkinan manifestasi ekstrakutan dan cenderung
untuk residif karena sering terjadi persintensi virus. Derajat
penularannya tinggi, tetapi karena patogenitas dan daya tahan terhadap
infeksi baik, maka infeksi ini sering berjalan tanpa gejala atau gejala
ringan, subklinis atau hanya local. ( Rassner Dermatologie Lehrbuch und
atlas, 1995)
B. Epidemiologi
Herpes
zoster dapat muncul disepanjang tahun karena tidak dipengaruhi oleh
musim dan tersebar merata di seluruh dunia, tidak ada perbedaan angka
penderita antara laki-laki dan perempuan, angka penderita meningkat
dengan peningkatan usia. Di negara maju seperti Amerika, penyakit ini
dilaporkan sekitar 6% setahun, di Inggris 0,34% setahun sedangkan di
Indonesia lebih kurang 1% setahun. Herpes zoster terjadi pada orang yang
pernah menderita varisela sebelumnya karena varisela dan herpes zoster
disebabkan oleh virus yang sama yaitu virus varisela zoster. Setelah
sembuh dari varisela, virus yang ada di ganglion sensoris tetap hidup
dalam keadaan tidak aktif dan aktif kembali jika daya tahan tubuh
menurun. Lebih dari 2/3 usia di atas 50 tahun dan kurang dari 10% usia
di bawah 20 tahun. Kurnia Djaya pernah melaporkan kasus hepes zoster
pada bayi usia 11 bulan.
Sedangkan
epidemiologi Herpes simpleks virus tipe II ditemukan pada wanita
pelacur 10x lebih tinggi daripada wanita normal. Sedangkan HSV tipe I
sering dijumpai pada kelompok dengan sosioekonomi rendah.
C. Klasifikasi
Herpes zoster dapat dibedakan menjadi :
a) Herpes zoster generalisata
Adalah
herpes yang unilateral dan segmental ditambah dengan penyebaran secara
generalisata berupa vesikel soliter dan terdapat umbilikasi.
b) Herpes zoster oftalmikus
Adalah
herpes zoster yang didalamnya terjadi infeksi cabang pertama nervus
trigeminus yang menimbulkan kelainan pada mata serta cabang ke 2 dan ke 3
yang menyebabkan kelainan kulit pada daerah persyarafan.
Berdasarkan perbedaan imunologi dan klinis, virus herpes simpleks dapat dibedakan menjadi dua tipe yaitu :
a) Virus herpes simpleks tipe 1
Menyebabkan infeksi herpes non genital, biasanya pada daerah mulut, meskipun kadang-kadang dapat menyerang daerah genital. Infeksi virus ini biasanya terjadi saat anak-anak dan sebagian besar seropositif telah didapat pada waktu umur 7 tahun.
b) Virus herpes simpleks tipe 2
Hampir secara eksklusif hanya ditemukan pada traktus genitalis dan sebagian besar ditularkan lewat kontak seksual.
Secara
periodik, virus ini akan kembali aktif dan mulai berkembangbiak,
seringkali menyebabkan erupsi kulit berupa lepuhan pada lokasi yang sama
dengan infeksi sebelumnya. Virus juga bisa ditemukan di dalam kulit
tanpa menyebabkan lepuhan yang nyata, dalam keadaan ini virus merupakan
sumber infeksi bagi orang lain.
D. Etiologi
Herpes zoster disebabkan oleh reaktivasi dari virus varicella zoster
. virus varicella zoster terdiri dari kapsid berbentuk ikosahedral
dengan diameter 100 nm. Kapsid tersusun atas 162 sub unit protein–virion
yang lengkap dengan diameternya 150–200 nm, dan hanya virion yang
terselubung yang bersifat infeksius. Infeksiositas virus ini dengan
cepat dihancurkan oleh bahan organic , deterjen, enzim proteolitik,
panas dan suasana Ph yang tinggi. Masa inkubasinya 14–21 hari.
a) Faktor Resiko Herpes zoster
1) Usia
lebih dari 50 tahun, infeksi ini sering terjadi pada usia ini akibat
daya tahan tubuhnya melemah. Makin tua usia penderita herpes zoster
makin tinggi pula resiko terserang nyeri.
2) Orang
yang mengalami penurunan kekebalan (immunocompromised) seperti HIV dan
leukimia. Adanya lesi pada ODHA merupakan manifestasi pertama dari
immunocompromised.
3) Orang dengan terapi radiasi dan kemoterapi.
4) Orang dengan transplantasi organ mayor seperti transplantasi sumsum tulang.

Secara umum, penyebab dari terjadinya herpes simpleks ini adalah sebagai berikut:
1) Herpes Virus Hominis (HVH).
2) Herpes Simplex Virus (HSV)
3) Varicella Zoster Virus (VZV)
4) Epstein Bar Virus (EBV)
5) Citamoga lavirus (CMV)
Namun
yang paling sering herpes simpleks disebabkan oleh virus herpes
simpleks tipe I dan tipe II. Cara penularan melalui hubungan kelamin,
tanpa melalui hubungan kelamin seperti : melalui alat-alat tidur,
pakaian, handuk,dll atau sewaktu proses persalinan/partus pervaginam
pada ibu hamil dengan infeksi herpes pada alat kelamin luar.

E. Manifestasi Kliniks
Herpes zoster
a) Gejala prodomal
§ Keluhan biasanya diawali dengan gejala prodomal yang berlangsung selama 1 – 4 hari.
§ Gejala
yang mempengaruhi tubuh : demam, sakit kepala, fatige, malaise, nusea,
rash, kemerahan, sensitive, sore skin ( penekanan kulit), neri, (rasa
terbakar atau tertusuk), gatal dan kesemutan.
§ Nyeri bersifat segmental dan dapat berlangsung terus menerus atau hilang timbul. Nyeri juga bisa terjadi selama erupsi kulit.
§ Gejala yang mempengaruhi mata : Berupa
kemerahan, sensitive terhadap cahaya, pembengkakan kelopak mata.
kekeringan mata, pandangan kabur, penurunan sensasi penglihatan dan lain
– lain.
b) Timbul erupsi kulit
§ Kadang terjadi limfadenopati regional
§ Erupsi
kulit hampir selalu unilateral dan biasanya terbatas pada daerah yang
dipersarafioleh satu ganglion sensorik. Erupsi dapat terjadi di seluruh
bagian tubuh, yang tersering di daerah ganglion torakalis.
§ Lesi
dimulai dengan macula eritroskuamosa, kemudian terbentuk papul–papul
dan dalam waktu 12–24 jam lesi berkembang menjadi vesikel. Pada hari
ketiga berubah menjadi pastul yang akan mengering menjadi krusta dalam
7–10 hari. Krusta dapat bertahan sampai 2–3 minggu kemudian mengelupas.
Pada saat ini nyeri segmental juga menghilang
§ Lesi baru dapat terus muncul sampai hari ke 4 dan kadang–kadang sampai hari ke 7
§ Erupsi kulit yang berat dapat meninggalkan macula hiperpigmentasi dan jaringan parut (pitted scar)
§ Pada lansia biasanya mengalami lesi yang lebih parah dan mereka lebih sensitive terhadap nyeri yang dialami.
Herpes simpleks
Masa
inkubasi berkisar sekitar 3-7 hari. Berdasarkan pernah tidaknya
seseorang kontak dengan Virus Herpes Simplex (HSV-2), infeksi Herpes
simpleks berlangsung dalam 3 fase, yakni:
1) Fase Infeksi (lesi) Primer, ditandai dengan:
§ Dapat terjadi tanpa gejala (asimptomatis)
§ Diawali dengan rasa panas, rasa terbakar dan gatal pada area yang terserang.
§ Kemudian
timbul vesikula (bintik-bintik) bergerombol, mudah pecah sehingga
menimbulkan perlukaan (mirip koreng) di permukaan kulit yang kemerahan
(eritematus), dan nyeri.
§ Selanjutnya dapat diikuti dengan demam, lemas sekujur tubuh (malaise) dan nyeri otot.
§ Terjadi pembesaran kelenjar getah bening di sekitar area yang terserang Herpes genitalis.
2) Fase Infeksi (lesi) Rekuren (kambuh).
Seseorang
yang pernah infeksi primer, dapat mengalami kekambuhan. Adapun
kekambuhan terjadi karena berbagai faktor dan dapat dipicu oleh beberapa
faktor pencetus, misalnya kelelahan fisik maupun psikis, alkohol,
menstruasi dan perlukaan setelah hubungan intim.
§ Pada
infeksi kambuhan (rekuren), gejala dan keluhan pada umumnya lebih
ringan. Gambaran penyakit bersifat lokal pada salah satu sisi bagian
tubuh (unilateral), berbentuk vesikuloulseratif (bercak koreng) yang
biasanya dapat hilang dalam 5 hingga 7 hari.
§ Sebelum muncul bercak berkoreng, didahului dengan rasa panas, gatal dan nyeri.
3) Fase Laten
Fase
ini berati penderita tidak ditemukan gejala klinis, tetapi HVS dapat
ditemukan dlm keadaan tidak aktif pada ganglion dorsalis
F. Patofisiologi
Herpes
zoster bermula dari Infeksi primer dari VVZ (virus varisells zoster)
ini pertama kali terjadi di daerah nasofaring. Disini virus mengadakan
replikasi dan dilepas ke darah sehingga terjadi viremia permulaan yang
sifatnya terbatas dan asimptomatik. Keadaan ini diikuti masuknya virus
ke dalam Reticulo Endothelial System (RES) yang kemudian mengadakan
replikasi kedua yang sifat viremianya lebih luas dan simptomatik dengan
penyebaran virus ke kulit dan mukosa. Sebagian virus juga menjalar
melalui serat-serat sensoris ke satu atau lebih ganglion sensoris dan
berdiam diri atau laten didalam neuron. Selama antibodi yang beredar
didalam darah masih tinggi, reaktivasi dari virus yang laten ini dapat
dinetralisir, tetapi pada saat tertentu dimana antibodi tersebut turun
dibawah titik kritis maka terjadilah reaktivasi dari virus sehingga
terjadi herpes zoster.
Patofisiologi herpes simpleks masih belum jelas, ada kemungkinan :
§ Infeksi
primer akibat transmisi virus secara langsung melalui jalur neuronal
dari perifer ke otak melalui saraf Trigeminus atau Offactorius.
§ Reaktivitas infeksi herpes virus laten dalam otak.
§ Pada neonatus penyebab terbanyak adalah HSV-2 yang merupakan infeksi dari secret genital yang terinfeksi pada saat persalinan.
G. Pemeriksaan Diagnostik
Pemeriksaan
diagnostic pada Herpes zoster. Tes diagnostic ini untuk membedakan dari
impetigo, kontak dermatitis dan herps simplex :
1) Tzanck Smear : mengidentifikasi virus herpes tetapi tidak dapat membedakan herpes zoster dan herpes simplex.
2) Kultur dari cairan vesikel dan tes antibody : digunakan untuk membedakan diagnosis herpes virus
3) Immunofluororescent : mengidentifikasi varicella di sel kulit
4) Pemeriksaan histopatologik
5) Pemerikasaan mikroskop electron
6) Kultur virus
7) Identifikasi anti gen / asam nukleat VVZ (virus varisela zoster)
8) Deteksi antibody terhadap infeksi virus
Pemeriksaan
penunjang untuk infeksi HSV (herpes simpleks virus dapat dilakukan
secara virologi maupun serologi, masing-masing contoh pemeriksaan
tersebut adalah sebagai berikut :
a) Virologi
1) Mikroskop
cahaya. Sampel berasal dari sel-sel di dasar lesi, apusan pada
permukaan mukosa, atau dari biopsi, mungkin ditemukan intranuklear
inklusi (Lipschutz inclusion bodies). Sel-sel yang terinfeksi dapat
menunjukkan sel yang membesar menyerupai balon (ballooning) dan
ditemukan fusi. Pada percobaan Tzanck dengan pewarnaan Giemsa atau
Wright, dapat ditemukan sel datia berinti banyak dan badan inklusi
intranuklear.
2) Pemeriksaan
antigen langsung (imunofluoresensi). Sel-sel dari spesimen dimasukkan
dalam aseton yang dibekukan. Kemudian pemeriksaan dilakukan dengan
menggunakan cahaya elektron (90% sensitif, 90% spesifik) tetapi,
pemeriksaan ini tidak dapat dicocokkan dengan kultur virus.
3) PCR,
Test reaksi rantai polimer untuk DNA HSV lebih sensitif dibandingkan
kultur viral tradisional (sensitivitasnya >95 %, dibandingkan dengan
kultur yang hanya 75 %). Tetapi penggunaannya dalam mendiagnosis infeksi
HSV belum dilakukan secara reguler, kemungkinan besar karena biayanya
yang mahal. Tes ini biasa digunakan untuk mendiagnosis ensefalitis HSV
karena hasilnya yang lebih cepat dibandingkan kultur virus.6
4) Kultur
Virus, Kultur virus dari cairan vesikel pada lesi (+) untuk HSV adalah
cara yang paling baik karena paling sensitif dan spesifik dibanding
dengan cara-cara lain. HSV dapat berkembang dalam 2 sampai 3 hari. Jika
tes ini (+), hampir 100% akurat, khususnya jika cairan berasal dari
vesikel primer daripada vesikel rekuren. Pertumbuhan virus dalam sel
ditunjukkan dengan terjadinya granulasi sitoplasmik, degenerasi balon
dan sel raksasa berinti banyak. Sejak virus sulit untuk berkembang,
hasil tesnya sering (-). Namun cara ini memiliki kekurangan karena waktu
pemeriksaan yang lama dan biaya yang mahal.
b) Serologi
Pemeriksaan
serologi ini direkomendasikan kepada orang yang mempunyai gejala herpes
genital rekuren tetapi dari hasil kultur virus negatif, sebagai
konfirmasi pada orang-orang yang terinfeksi dengan gejala- gejala herpes
genital, menentukan apakah pasangan seksual dari orang yang
terdiagnosis herpes genital juga terinfeksi dan orang yang mempunyai
banyak pasangan sex dan untuk membedakan dengan jenis infeksi menular
sexual lainnya. Sample pada pemeriksaan serologi ini diambil dari darah
atau serum. Pemeriksaannya dapat berupa :
1) ELISA,
Dasar dari pemeriksaan ELISA adalah adanya ikatan antara antigen dan
antibodi, dimana antigen berasal dari suatu konjugat igG dan antibodi
berasal dari serum spesimen. Setelah spesimen dicuci untuk membersihkan
sample dari material (HRP) kemudian diberi label antibodi IgG konjugat.
Konjugat ini dapat mengikat antibodi spesifik HSV-II. komplek imun
dibentuk oleh ikatan konjugat yang ditambah dengan Tetramethylbenzidine
(TMB) yang akan memberikan reaksi berwarna biru. Asam sulfur ditambahkan
untuk menghentikan reaksi yang akan memberikan reaksi warna kuning.
Pembacaan reaksi dilakukan dengan mikrowell plate reader ELISA dengan
panjang gelombang 450 nm.
Interpretasi hasil:
§ Jika
terdapat antibodi HSV-II berarti pernah terinfeksi HSV-II, virus dorman
didalam nervus sakralis dan pasien sedang menderita herpes genitalis.
§ Jika
antibodi HSV-II tidak ada berarti 95-98% anda tidak menderita herpes
genital kecuali anda baru saja terinfeksi HSV-II karena antibodi baru
akan terbentuk 6 minggu kemudian, bahkan ada beberapa individu (1
diantara 5) baru mampu membentuk antibodi tersebut setelah 6 bulan, oleh
karena itu lebih baik mengulang pemeriksaan 6-8 minggu kemudian.
§ Jika
terdapat antibodi HSV-I berarti anda mengalami infeksi HSV-I. Antibodi
ini tidak bisa mendeteksi virus yang dorman. Pada sebagian besar orang
(>90%) virus berada dalam syaraf mulut dan mata. Beberapa orang yang
mempunyai infeksi HSV-I pada genital dapat mempunyai antibodi dari
infeksi HSV-I pada daerah genital.
§ Jika
tidak terdapat antibodi HSV-I dan HSV-II, berarti anda tidak terinfeksi
HSV-I maupun HSV-II tetapi suatu ketika anda mungkin dapat terinfeksi.
Namun tidak menutup kemungkinan bahwa anda baru saja terinfeksi tetapi
belum terbentuk antibodi.
§ Pada
infeksi primer, antibodi HSV-I dan II dapat terdeteksi pada hari-hari
awal setelah onset dari penyakit. Serokonversi terhadap kandungan
antibodi Ig M dan IgG diperlukan sebagai deteksi adanya infeksi primer,
sebagai tambahan antibodi IgA spesifik juga dapat terdeteksi mengikuti
terbentuknya antibodi IgM dan IgG. Ketika infeksi berjalan, antibodi IgM
dan IgA belum terdeteksi beberapa minggu-bulan ketika individu tersebut
telah mempunyai antibodi IgG yang menetap dalam tubuhnya untuk seumur
hidup dan dalam titer yang tinggi (gambar A). Pola serologis yang lain
membuktikan kandungan IgG, IgM dan IgA pada kasus reaktivasi dari
infeksi laten atau periode reinfeksi (gambar B). Sebagian besar serum
sampel diambil dalam waktu 7-10 hari setelah terinfeksi menunjukkan
peningkatan antibodi IgG yang signifikan. Peningkatan kadar antibodi IgA
juga sering ditemui, peningkatan serokonversi IgA pada kasus dimana
juga terjadi peningkatan kadar IgG menunjukkan bahwa serum sampel secara
serologik terinfeksi HSV.
2) Western
Blot Test, merupakan test yang sangat akurat untuk mendeteksi HSV,
namun harganya lebih mahal dibandingkan tes-tes yang lain dan
membutuhkan waktu yang lebih lama untuk mengintepresentasikannya. Test
ini merupakan metoda gold standard dalam pemeriksaan antibodi. Tes ini
hanya digunakan sebagai referensi dan konfirmasi apabila tes dengan
ELISA menunjukkan hasil yang meragukan. Test ini memiliki ketelitian
untuk menyimpulkan secara spesifik bahwa sample benar-benar mengandung
antibodi terhadap protein tertentu dari virus.
3) Biokit
HSV-II, merupakan tes untuk mendeteksi antibodi HSV tipe II. Tes ini
merupakan tes yang cepat, hanya kira-kira membutuhkan waktu 10 menit dan
hasilnya juga cepat ditunjukkan. Hasil positif ditunjukkan dengan dua
warna merah yang lebih tipis bila dibandingkan dengan kontrol. Jika
antibodi HSV-II tidak ada, maka hanya tampak satu warna merah. Jika
hanya mengandung antibodi HSV-I maka hanya akan ada satu tanda merah.
Jika tidak terdapat tanda merah maka tes tersebut tidak valid dan harus
diulang.

H. Penatalaksaan Herpes.
Penatalaksanaan Herpes zoster
a) Pengobatan
1) Pengobatan topical
§ Pada stadium vesicular diberi bedak salicyl 2% atau bedak kocok kalamin untuk mencegah vesikel pecah
§ Bila
vesikel pecah dan basah, diberikan kompres terbuka dengan larutan
antiseptik atau kompres dingin dengan larutan burrow 3 x sehari selama
20 menit
§ Apabila
lesi berkrusta dan agak basah dapat diberikan salep antibiotik
(basitrasin / polysporin ) untuk mencegah infeksi sekunder selama 3 x
sehari.
2) Pengobatan sistemik
§ Drug
of choice- nya adalah acyclovir yang dapat mengintervensi sintesis
virus dan replikasinya. Meski tidak menyembuhkan infeksi herpes namun
dapat menurunkan keparahan penyakit dan nyeri. Dapat diberikan secara
oral, topical atau parenteral. Pemberian lebih efektif pada hari pertama
dan kedua pasca kemunculan vesikel. Namun hanya memiliki efek yang
kecil terhadap postherpetic neuralgia.
§ Antiviral lain yang dianjurkan adalah vidarabine (Ara – A, Vira – A) dapat diberikan lewat infus intravena atau salep mata.
§ Kortikosteroid
dapat digunakan untuk menurunkan respon inflamasi dan efektif namun
penggunaannya masih kontroversi karena dapat menurunkan penyembuhan dan
menekan respon immune.
§ Analgesik non narkotik dan narkotik diresepkan untuk manajemen nyeri dan antihistamin diberikan untuk menyembuhkan priritus.
b) Penderita dengan keluhan mata
Keterlibatan
seluruh mata atau ujung hidung yang menunjukan hubungan dengan cabang
nasosiliaris nervus optalmikus, harus ditangani dengan konsultasi
opthamologis. Dapat diobati dengan salaep mata steroid topical dan
mydriatik, anti virus dapat diberikan
c) Neuralgia Pasca Herpes zoster
§ Bila
nyeri masih terasa meskipun sudah diberikan acyclovir pada fase akut,
maka dapat diberikan anti depresan trisiklik ( misalnya : amitriptilin
10 – 75 mg/hari)
§ Tindak lanjut ketat bagi penanganan nyeri dan dukungan emosional merupakan bagian terpenting perawatan
§ Intervensi
bedah atau rujukan ke klinik nyeri diperlukan pada neuralgi berat yang
tidak

Pada prinsipnya, penanganan dari infeksi Herpes Simpleks Virus (HSV) ada 2 macam, yaitu:
1) Terapi Spesifik;
a) Infeksi primer
§ Topikal
: Penciclovir krim 1% (tiap 2 jam selama 4 hari) atau Acyclovir krim 5%
(tiap 3 jam selama 4 hari). Idealnya, krim ini digunakan 1 jam setelah
munculnya gejala, meskipun juga pemberian yang terlambat juga
dilaporkan masih efektif dalam mengurangi gejala serta membatasi
perluasan daerah lesi. (Rekomendasi FDA & IHMF)
§ Sistemik
: Valacyclovir tablet 2 gr sekali minum dalam 1 hari yang diberikan
begitu gejala muncul, diulang pada 12 jam kemudian, atau Acyclovir
tablet 400 mg 5 kali sehari selama 5 hari, atau Famciclovir 1500 mg
dosis tunggal yang diminum 1 jam setelah munculnya gejala prodromal.
b) Infeksi Rekuren
Terapi
rekuren ditujukan untuk mengurangi angka kekambuhan dari herpes
genitalis, dimana tingkat kekambuhan berbeda pada tiap individu,
bervariasi dari 2 kali/tahun hingga lebih dari 6 kali/tahun. Terdapat 2
macam terapi dalam mengobati infeksi rekuren, yaitu terapi episodik dan
terapi supresif.
Ø Terapi Episodik:
§ Acycovir, 400 mg p.o 3 x/hr, 5 hr, atau 800 mg 2 x/hr, 5 hr, atau 800 mg p.o 3 x/hr,3 hr
§ Valacyclovir, 500 mg p.o 2 x/hr 3 hr, atau 1 gr p.o 1x/hr, 5 hr
§ Famciclovir, 125 mg p.o 2 x/hr,5 hr, atau 1 gr p.o 2 x/hr,1 hr
Ø Terapi Supresif:
§ Acyclovir 400 mg p.o 2 x/hr selama 6 th, atau
§ Famciclovir 250 mg p.o 2 x/hr selama 1 th, atau
§ Valacyclovir 500 mg p.o 1x/hr selama 1 th, atau
§ Valacyclovir 1 gr p.o 1x/hr selama 1 th
2) Terapi Non-Spesifik;
Pengobatan
non-spesifik ditujukan untuk memperingan gejala yang timbul berupa
nyeri dan rasa gatal. Rasa nyeri dan gejala lain bervariasi, sehingga
pemberian analgetik, antipiretik dan antipruritus disesuaikan dengan
kebutuhan individu. Zat-zat pengering yang bersifat antiseptic juga
dibutuhkan untuk lesi yang basah berupa jodium povidon secara topical
untuk mengeringkan lesi, mencegah infeksi sekunder dan mempercepat waktu
penyembuhan. Selain itu pemberian antibiotic atau kotrimoksasol dapat
pula diberikan untuk mencegah infeksi sekunder
Tujuan
dari terapi tersebut masing-masing adalah untuk mempercepat proses
penyembuhan, meringankan gejala prodromal, dan menurunkan angka
penularan.(

BAB III
ASUHAN KEPERAWATAN HERPES
A. Pengkajian
1. Biodata
a. Identitas Pasien
Di
dalam identitas hal-hal yang perlu di kaji antara lain nama pasien,
alamat pasien, umur pasien biasnya kejadian ini mencakup semua usia
antara anak-anak sampai dewasa, tanggal masuk ruma sakit penting untuk
di kaji untuk melihat perkembangan dari pengobatan, penanggung jawab
pasien agar pengobatan dapat di lakukan dengan persetujuan dari pihak
pasien dan petugas kesehatan.
2. Riwayat Kesehatan
a. Keluhan Utama
Gejala
yang sering menyebabkan penderita datang ke tempat pelayanan kesehatan
adalah nyeri pada lesi yang timbul dan gatal-gatal pada daerah yang
terkena pada fase-fase awal baik pada herpes zoster maupun simpleks.
b. Riwayat penyakit Sekarang
Penderita
merasakan nyeri yang hebat, terutama pada area kulit yang mengalami
peradangan berat dan vesikulasi yang hebat, selain itu juga terdapat
lesi/vesikel perkelompok dan penderita juga mengalami demam.
c. Riwayat penyakit keluarga
Tanyakan kepada penderita ada atau tidak anggota keluarga atau teman dekat yang terinfeksi virus ini.

Sering diderita kembali oleh klien yang pernah mengalami penyakit herpes simplek atau memiliki riwayat penyakit seperti ini

respon pasien terhadap penyakit byang diderita serta peran dalam
keluarga dan masyarakat, respon dalam keluarga maupun masyarakat.
3. Pola Kehidupan
a. Aktivitas dan Istirahat
Apakah pasien mengeluh merasa cemas, tidak bisa tidur karena nyeri, dan gatal.
b. Pola Nutrisi dan Metabolik
Bagaimana pola nutrisi pasien, apakah terjadi penurunan nafsu makan, anoreksia.
c. Pola Aktifitas dan Latihan
Dengan adanya nyeri dan gatal yang dirasakan, terjadi penurunan pola akifitas pasien.
d. Pola Hubungan dan peran
Klien akan sedikit mengalami penurunan psikologis, isolasi karena adanya gangguan citra tubuh.

d. Hidung (Penciuman)
Posisi
sektum naso tepat ditengah, tidak terdapat secret, tidak terdapat lesi,
dan tidak terdapat hiposmia. Anosmia, parosmia, kakosmia.
e. Telinga (Pendengaran)
Ø Inspeksi
v Daun telinga : tidak terdapat lesi, kista epidemoid, dan keloid.
v Lubang telinga : tidak terdapat obstruksi akibat adanya benda asing.
Ø Palpasi
Tidak terdapat edema, tidak terdapat nyeri tekan pada otitis media dan mastoidius.
Ø Pemeriksaan pendengaran
v Test audiometric : 26 db (tuli ringgan)
v Test weber : telinga yang tidak terdapat sumbatan mendengar lebih keras.
v Test rinne : test (-) pada telinga yang terdapat sumbatan
f. Mulut dan gigi
Mukosa bibir lembab, tidak pecah-pecah, warna gusi merah muda, tidak terdapat perdarahan gusi, dan gigi bersih.
g. Leher
Posisi trakea simetris, tidak terdapat pembesaran kelenjar tiroid, tidak ada pembesaran vena jugularis, tidak ada nyeri tekan.
h. Thorak
Ø Bentuk : simetris
Ø Pernafasan : regular
Ø Tidak terdapat otot bantu pernafasan

j. Reproduksi
Pada
pemeriksaan genitalia pria, daerah yang perlu diperhatikan adalah
bagianglans penis, batang penis, uretra, dan daerah anus. Sedangkan pada
wanita,daerah yang perlu diperhatikan adalah labia mayora dan minora,
klitoris, introitus vagina, dan serviks
Jika
timbul lesi, catat jenis, bentuk, ukuran / luas,warna, dan keadaan
lesi. Palpasi kelenjar limfe regional, periksa adanyapembesaran; pada
beberapa kasus dapat terjadi pembesaran kelenjar limferegional

Tidak terdapat luka dan spasme otot.
l. Integument
Ditemukan adanya vesikel-vesikel berkelompok yang nyeri,edema di sekitar lesi,dan dapat pula timbul ulkus pada infeksi sekunder.

BAB IV
KESIMPULAN
§ Herpes
zoster adalah penyakit yang disebabkan oleh reaktivasi virus Varicella –
Zoster yang sifatnya localized, dengan ciri khas berupa nyeri
radikuler, unilateral, dan gerombolan vesikel yang tersebar sesuai
dermatom yang diinervasi satu ganglion saraf sensoris.
§ Herpes
simpleks adalah infeksi akut yg disebabkan oleh virus herpes simpleks
(virus herpes hominis) tipe I atau tipe II yang ditandai oleh adanya
vesikel yang berkelompok diatas kulit yang sembab dan eritematosa pada
daerah dekat mukokutan, sedangkan infeksi dapat berlangsung baik primer
maupun rekurens
§ Herpes
zoster disebabkan oleh reaktivasi dari virus varicella zoster . virus
varicella zoster terdiri dari kapsid berbentuk ikosahedral dengan
diameter 100 nm. Kapsid tersusun atas 162 sub unit protein–virion yang
lengkap dengan diameternya 150–200 nm, dan hanya virion yang terselubung
yang bersifat infeksius. Infeksiositas virus ini dengan cepat
dihancurkan oleh bahan organic , deterjen, enzim proteolitik, panas dan
suasana Ph yang tinggi. Masa inkubasinya 14–21 hari
§ Secara umum, penyebab dari terjadinya herpes simpleks ini adalah sebagai berikut
1) Herpes Virus Hominis (HVH).
2) Herpes Simplex Virus (HSV)
3) Varicella Zoster Virus (VZV)
4) Epstein Bar Virus (EBV)
5) Citamoga lavirus (CMV)
